New Patient Registration

New Patients

New patients should arrive at least 15 minutes before their appointment to fill out forms and provide insurance details. We include our new patient registration form online for first-time patients to print, complete, and bring on the day of their appointment.

Patient Emergencies

  • A patient of record who experiences a dental emergency will be scheduled promptly.
  • We will attempt to see you on the same day whenever possible.
  • If you have an emergency, please call the office for assistance.

Cancellations

  • When you schedule an appointment, Elevation Dental and Aesthetics has reserved this time just for you. This is how we are able to see patients on time and get them in and out on time.
  • If you are unable to keep an appointment, we kindly ask that you provide at least 24 hours’ notice so we may offer the appointment to another patient who needs it.
