There are no age restrictions, employment requirements, or limits based on your current dental health. Anyone can join. Members typically save hundreds (and sometimes thousands) on their annual dental costs while still receiving the comprehensive care they need.

Yearly enrollment: $645 per individual

What’s included:

  • Three comprehensive exams per year, including standard x-rays (3D imaging not included)
  • Three cleanings annually (for non-periodontal patients)
  • Three fluoride treatments
  • A discount of up to 15% on any additional dental services
  • Special pricing on oral surgery and periodontal procedures
  • No deductibles, annual caps, or waiting periods to worry about

Additional details:

  • Full payment is required at the time of your appointment to apply the membership discount
  • Membership benefits are personal and non-transferable, and cannot be stacked with other promotions or discount offers
  • This plan is an office-based savings program available exclusively at Elevation Dental and Aesthetics — it is not insurance
  • Memberships cannot be refunded, and any benefits left unused at year’s end will not carry over
  • Discount tier: 15% when paying by cash or check; 10% when paying by credit card

Periodontal Care Membership Plan

Yearly enrollment: $785 per individual

What’s included:

  • Three comprehensive exams per year, including standard x-rays (3D imaging not included)
  • Four periodontal maintenance visits annually
  • Three fluoride treatments
  • A discount of up to 15% on any additional dental services (see payment notes below)
  • Special pricing on oral surgery and periodontal procedures
